dc.description.abstractThis presentation explores the positive effect the Care Transformation Collaborative of Rhode Island (CTC) has had on in-home care of the elderly across the state. CTC helps existing community health teams develop new systems of care coordination, improve the communication between caretakers and patients and assist coordinators to measure success in effective ways. CTC is a patient-centered medical home initiative managed by UMass Medical School.
